For those who need to verify their volunteer hours for a school program, this form will help you track your hours, and if needed, can be signed and verified by the Community Coordinator.

Though you are expected to check in with the Coordinator from time to time, a proper follow up will happen roughly every 4 and 8 months. This means both pairs will be contacted and the form below will be filled out with both individuals. This is to ensure goals are being met and to ensure satisfaction for the pair.
